M&D Preparation
Deliverable: Email completed M&D Template, Metrics Plan, and Surveys to NCSMH evaluation team.
- The M&D Template is each grantee's empty excel template that includes numeric goals for each metric + assessment tool names. Submit this to the NCSMH without the signed attestation or report period data.
- The Metrics Plan is a one-time, narrative report that complements the M&D template. Use this document to describe, in detail, how you will collect, assess, and define the metrics you will submit. Do not use this form to describe goal calculation or report actual data.

- Sample Satisfaction Survey (items 1, 2, 8, 9, & 10 assess satisfaction)
- Scoring option 1: Average items 1, 2, 8, 9, 10, anyone >4 is "satisfied."
- Scoring option 2: A response of 4 or 5 on item 10 is "satisfied."
- Other scoring/definition methods can be used, describe in Metrics Plan.
- Items 3-7 can be used to monitor outcomes. Items can be asked at pre-intervention in absence of other available standardized measured.
- After your one-on-one meeting with the NCSMH, submit your M&D template (with goals and measures added), Metrics Plan, and all Satisfaction/Outcome surveys used in reporting that are not in the Outcome Measure Menu by the agreed-upon date via email. The NCSMH will review your documents and get back to you ASAP either with your final M&D template to use for reporting or with follow-up questions. The attestation does not need to be signed to finalize your M&D template.
M&D Report Period 1
Deliverable: Submit M&D Report Period 1 data and Progress Report #2 by COB Aug 1, 2024 via CHRC SmartSheets Portal.
- The attestation needs to be signed to submit your M&D reports.
- Include numeric values for all relevant metric. Anything not relevant is greyed out. All other report period 1 metric should be populated with values, even if they are 0. We anticipate many 0s for report period 1.
- If you have received a copy of your final M&D template from the NCSMH, use that to submit your data. If you have not received it, use the most updated copy of the template available to you.
- If your first M&D report has errors, the CHRC team will contact you about submitting a revised copy.
